Request for Confirmation of Customer Information

We hope this correspondence finds you well, and thank you for your continued support of our creditcard.

We are writing to you today to ask you to provide confirmation once again that the information we hold about you is up to date and correct. This is being done as part of efforts to combat money laundering and terrorist financing, required by Japanese government and the international community, and to provide peace of mind and convenience when you use your credit card.
Please submit (by upload or mail) your residence card information to us by one of the following methods of submission.
If you have a Special Permanent Resident Certificate, please submit it as described in (2) Procedure for Submission by Postal Mail.

[How to Submit]

Please use either procedure (1) or (2) to complete submission by the deadline stated in the letter you received in the mail.
For some customers, we only allow submission by (2) Procedure for Submission by Postal Mail. If this applies to you, please submit by postal mail.

1. 1. Procedure for Submission by Smartphone (no need to send postal mail). This is the recommended method of submission.
      
   2. Procedure for Submission by Postal Mail

      Please fill out the Customer Information Confirmation Form included with the letter sent to you in the mail, and return it to us using the return envelope provided.
      If you use the above (1) Procedure for Submission by Smartphone, you do not need to submit anything by mail.

• The Customers who do not submit or return the form to us may be sent reminder letters.
• If you do not submit the form, you may not be able to use your card.

The Customer Information Confirmation Form described in the letter sent to you in the mail is not posted on this site.
